In order to gain optimum benefit from your consultation we ask that you send the following materials BEFORE your scheduled consultation. This will allow the doctor to preview your situation and reduce the amount of time you are in consultation (therefore reducing your cost).
Intake form (download as pdf)-- we must have this to schedule consult
Laboratory results (within past year)
Pathology reports (if surgery or biopsy has been done)
Imaging reports (X-rays, CT, MRI, etc.)
Phone consultations are scheduled during regular office hours. Rates for consultations are outlined below. Payment must be in the form of a credit card. Please call toll free 1-877-931-1167 for more information.
Phone consultation rates:
$120.00 per hour
$60.00 per ½ hour
$30.00 per 15 minutes
The initial phone consultation is between 45 minutes-1.5 hours. Follow-up consultations usually take 30 minutes or less. These times vary according to the complexity of your condition. If you have questions regarding rates or billing, please call us for more information.
Your insurer may cover consultation. for phone consults, we do not process billing claims directly from our office. We will be happy to provide you with the proper forms needed for reimbursement by your insurer. Please call our office if you need help determining your coverage.
